Prescriptions

The law requires we have a valid prescription on file before we ship your mask, machine and/or humidifier. You do not need a new prescription to fulfill this requirement and can send us your original prescription, provided it has not expired. 

We provide many easy ways to get your prescription to Lakeland Sleep Centers:

• We can request your prescription for you! When you checkout, just let us know you would like us to request your prescription.  Please add your doctor's contact information in the special instructions box.  We will contact your doctor on your behalf.
• Fax your prescription to 763-576-1660, you may send it yourself or ask your doctor's office to send the prescription to us.
• Email your prescription to our store dme@lhssleep.com.

If you are planning to visit your doctor soon, you can ask them to send your prescription to our offices.
